• Computerized National Identity Card for 18 years and above or computerized form B along with Guardian CNIC (Father or Mother) for less than 18 years of age.
• One set of attested documents of above mentioned documents.
• 3 passport size colored photos dully attested.
• Note: Old National Identity Card will not be accepted.
